Maturity Levels
All UX work (each Jira Story) should be marked with a Maturity Level (ML) to indicate its completeness.
The Maturity Label should appear on the work - ie User Journey, Wireframe, UI Design, Prototype.

Process
If a story passes to UX Maturity Level 3, 4 or 5 and then requires amends it immediately reverts to Maturity Level 2, is worked on and progresses forward.
